Death and Burial: A Checklist
Preparation
1. Discuss final wishes with family members/close friends:

_____ Funeral Service or Memorial Service _____Burial plot
_____ Mortuary choice
2. Prepare a Will with an attorney to designate distribution of your property at the time of death.
3. Consider a Power of Attorney to designate someone to administer financial/legal matters during your lifetime.
4. Discuss Living Will or Health Care Power of Attorney with family members (for medical decisions during lifetime).
5. Notify private physician of Living Will or Health Care Power of Attorney, to be placed in your medical records.
Information for Survivors
When Death Occurs
1. Call Pastor Carlson 612-729-8358.
2. Then, if death occurs at home, call Medical Examiner (Hennepin County 612-347-2125; Ramsey County 651-224-7627).
3. Call appropriate family members; perhaps inquire as to scheduling issues for funeral/memorial.
4. If death occurs at home, call designated mortician for removal of body. (At hospital or nursing home this is done by staff.)
5. Arrange for a meeting with the morticians, inviting Pastor Johnson to be present to offer assistance in decision-making.
Arrangements for Burial/Memorial Service
1. Meet with Pastor Johnson to plan funeral/memorial service (select hymns, solos, scripture).
2. Decide upon place and time for service: Holy Trinity or designated mortuary.
3. Reviewal - yes or no.
4. Supply mortuary with list of surviving relatives for memorial folder. and for newspaper obituary. (Mortuary places obituary in newspaper.)
5. Supply mortuary with clothing for the deceased if there is reviewal.
6. Advise mortuary as to pre-purchased plot, grave marker, urn.
